Terms and Conditions
This form explains the terms and conditions of participating in virtual health and wellness coaching services with Pingel Productions, LLC ("PP") & Dr. Tricia Pingel.("DRP") By signing this agreement, the Client agrees to the terms and conditions set forth below:
- I understand that although Dr. Tricia Pingel is a licensed physician in the state of Arizona, the services offered by Tricia Pingel are virtual wellness educational consultations in the capacity of a health coach, not as my physician.
- I understand that the wellness education consultations do not claim to treat, cure, or diagnose any specific disease or medical condition.
- I understand that consultations are designed to enhance my knowledge of health as it relates to food, dietary supplements, nutrition & lifestyle and that all recommendations should be reviewed by my primary care doctor before implementing
- I understand that I should not add, discontinue or change any medications/supplements without first consulting my primary care physician for approval.
- I acknowledge and understand that individual results may vary and no warranties or representations will be made with respect to individual results.
Release:
I release PP & DRP from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, against PP & DRP, arising from my participation in, or otherwise with respect to, the wellness coaching educational sessions.
Confidentiality
- I acknowledge that PP & DRP will keep my information private, and will not share my information with any third party, for marketing purposes, or for any other reason without consent of the client.
- I understand that all of the information I receive from the wellness consult is for my personal use and may be shared only with my immediate family and healthcare team.
- I understand that no part of this information may be reproduced, stored in or introduced into a retrieval system, or transmitted, in any form or by any means including, but not limited to, electronic, mechanical, photocopy, recording without the prior written permission of PP & DRP.
Fee and Cancellations
Payments and Cancellations
- Fees must be paid in advance of scheduling an appointment.
No Show and Cancellation policy
- In the event that the Client does not show for an appointment and provides no prior notice - the entire fee will be retained.
- Reschedules prior to 48 hours of appointment time are not penalized. We understand things happen, so please communicate with us as soon as possible to avoid fees.
- Deposited fees are good for 3 months to be used for an appointment. In the event that no appointment is scheduled, the paid fee will expire and can no longer be used. Please book your appointment as soon as possible to avoid this situation.
- In the event that you choose to not use the paid fees for an appointment, you may transfer these funds (within 90 days of purchase) to other services offered by Dr. Tricia Pingel. This could be membership fees, supplement credit or concierge medical services (if available).
- PP & DRP reserves the right to retain the entire fee for Cancellations within 24 hours of appointment time.
- PP & DRP reserves the right to refuse future appointment booking in the event of chronic last minute rescheduling and/or cancellations.
