1. I understand that although Dr. Tricia Pingel is a licensed physician in the state of Arizona, the material offered during this program is educational in nature and is not to be taken as medical advice.
2. I understand that the wellness education contained within this programs does not claim to treat, cure, or diagnose any specific disease or medical condition.
3. I understand that by entering into this program, that it does NOT constitute a doctor-patient relationship with Dr. Tricia Pingel and that all recommendations gained from this educational material should be discussed in full with my medical provider before implementing.

Privacy
This program involves group Q and A calls and some participants may choose to voluntarily share their health history. By signing this agreement you are agreeing to maintaining the privacy of others in the group and not share others stories or experiences without direct written consent. The choice to share your personal history or details, name, or use the camera function during these calls is up to your discretion and is not required for participation. Any information that you share during these calls or in community forums is voluntary on your part.
